I understand that as the purchaser of the program(s) indicated above, I am obligated to pay the full amount as outlined. I understand that failure to attend classes will NOT result in a refund or cessation of payments. I understand that I am purchasing a membership with a beginning and ending date, and that it is my responsibility to ensure regular class attendance for the duration of this membership time. Rising Sun Martial Arts is not responsible for issuing refunds or adding additional membership time if classes or membership time is missed. Rising Sun Martial Arts reserves the right to terminate the membership of any participant when it is considered to be in the best interest of either the student in question, the school, or other participants. I hereby release Rising Sun Martial Arts from all claims of damages arising from any accidents or injuries sustained during the normal participation of the program.
